Lower limb ulcer is a condition commonly seen to cause pain and social distress. It affects approximately 4% of the people above 65 years [1]. These wounds can be either acute or chronic depending on their duration since onset. Acute wounds heal within a predicted time frame. Wounds that do not heal in a timely and predicted period of three months are classified as chronic wounds. Diabetes, venous diseases, neuropathy, arterial diseases, and trauma are the common causes of ulcer formation [1]. Due to multiple causative factors of lower limb ulcers, it requires a multidisciplinary approach to assess the etiopathogenesis of the disease, and establish the diagnosis. The management of lower limb ulcers must include the history, clinical examination of the ulcer and the entire lower limb, relevant investigations, and treatment should be planned after identifying the cause of the ulcer. The efficient healing of a skin wound is something that most of us take for granted but is essential for surviving day-to-day knocks and cuts, and is absolutely relied on clinically whenever a patient receives surgical intervention. However, the management of a chronic wound - defined as a barrier defect that has not healed in 3 months - has become a major therapeutic challenge throughout the Western world, and it is a problem that will only escalate with the increasing incidence of conditions that impede wound healing, such as diabetes, obesity and vascular disorders. Despite being clinically and molecularly heterogeneous, all chronic wounds are generally assigned to one of three major clinical categories: leg ulcers, diabetic foot ulcers or pressure ulcers. Background Exact diagnosis of lower-extremity ulcers is essential for successful ulcer management, and it often needs a multidisciplinary approach. The most common aetiologies for lower-extremity leg ulcers are venous, arterial, mixed venous and arterial, diabetic and pressure ulcers. However, atypical wounds comprise approximately 20% of chronic wounds and should be kept in mind during differential diagnosis. Aim This review aims to provide clinicians with an overview of the different aetiologies of leg ulcer by describing the clinical characteristics of each aetiology. It also aims to provide tools for health-care providers when assessing a lower-extremity ulcer patient. Findings Ulcers with more than one aetiology are not uncommon. This awareness is increasing, and a holistic, systematic and checklist-based assessment is strongly recommended for each wound patient. One diagnosis does not exclude another. ABSTRACTThe management of patients with trophic ulcers and their consequences is difficult not only because it is a recurrent and recalcitrant problem but also because the pathogenesis of the ulcer maybe different in each case. Methodically and systematically evaluating and ruling out concomitant pathologies helps to address each patient′s specific needs and hence bring down devastating complications like amputation.
